ACSD says the horse-drawn vehicle, operated by 54-year-old Ida J. Schwartz of rural Monroe, was traveling West on S.R. 124.
Deputies say the buggy was slowing down to enter a driveway when a Hyundai Minivan driven by 34-year-old Zachary R. Knebel of Wilshire, Ohio rear-ended the Schwartz's vehicle.
They said Schwartz and two girls, who were also on the buggy were all ejected.
Schwartz was pronounced dead at the scene, according to deputies.
The two girls were transported to Adams Memorial Hospital for medical treatment.
Investigators said Knebel was wearing his seat belt and declined medical treatment at the scene.
The crash is still under investigation.
